Terms of Service

1. Acceptance

By accessing or using The Site Report (“the Service”), you agree to be bound by these Terms of Service. If you do not agree, do not use the Service.

2. Eligibility

The Service is intended exclusively for licensed anesthesia professionals — physician anesthesiologists (MD/DO), Certified Registered Nurse Anesthetists (CRNAs), and Certified Anesthesiologist Assistants (AAs) — with a valid, active National Provider Identifier (NPI). You must verify your credentials before accessing report cards.

3. Give-to-Get Model

Access to the full database of report cards requires submission of at least one report card of your own. By submitting a report card, you grant The Site Report a non-exclusive, royalty-free license to display your anonymized submission to other verified members.

4. Your Content

You are solely responsible for the accuracy and legality of the content you submit. By submitting a report card, you represent that:

• You personally worked at or with the practice you are reviewing.
• The information you provide is, to the best of your knowledge, accurate.
• Your submission does not contain false statements of fact, private information about identifiable individuals, or content that violates any applicable law.

5. Prohibited Conduct

You agree not to:

• Submit false or defamatory statements about any person or organization.
• Impersonate another provider or use a fraudulent NPI.
• Submit reviews on behalf of another person or entity (including employers, agencies, or competitors).
• Use the Service for commercial purposes, including recruiting or soliciting other users.
• Attempt to reverse-engineer, scrape, or otherwise extract data from the Service in bulk.

6. Content Moderation

We reserve the right to remove, edit, or hide any report card that violates these Terms, appears to be submitted in bad faith, or is flagged by the community as inappropriate. Flagged reports are reviewed by site administrators and may be removed at our discretion.

7. Disclaimer of Warranties

The Service and all content within it are provided “as is.” Report cards reflect the opinions and experiences of individual users and are not verified for factual accuracy. The Site Report makes no representations or warranties about the completeness, reliability, or accuracy of any report card. You should not rely solely on this information when making employment or contracting decisions.

8. Limitation of Liability

To the maximum extent permitted by law, The Site Report and its operators shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of the Service or reliance on any content herein.

9. Changes to These Terms

We may update these Terms from time to time. Continued use of the Service after any changes constitutes acceptance of the revised Terms. The effective date above reflects the date of the most recent revision.
